Clients will be required to settle ALL costs incurred on the day of treatment. If you are claiming treatment under WORKCOVER/TAC/COMCARE/NDIS or other third party, you agree to accept responsibility for payment of treatment received, in the event your claim for treatment is declined.
MY PHYSIO REQUIRES 24 HOURS’ ADVANCE NOTICE TO CANCEL AN APPOINTMENT, HYDROTHERAPY OR CLINICAL REFORMER CLASSES. FAILURE TO PROVIDE ADEQUATE NOTICE WILL INCUR A NON-ATTENDANCE FEE.
MYPHYSIO may use your information to communicate with other medical practitioners, claims officer and employer (as applicable).
